Frank, under stress at home now that he and Catherine have separated, is sent to Arkansas to investigate allegations of child abuse at a day care center. Although he questions why the Group would be interested in such a case, he is told that he needs to confront pure evil. Soon after he arrives, he confronts Lara Means, a psychiatrist investigating the case from the opposite angle, only to discover that they were both sent by Millennium. When a young boy turns up dead during nap time, the owner of the day care center is arrested and questioned. Although Frank has his suspicions about who the real killer is, he soon finds himself behind bars after a young girl accuses him of hitting her during an interview. Somehow, he has to convince a sheriff up for re-election in an angry community that both he and the center owner are innocent.

A taxi picks up a middle-aged man standing out in the rain. Once in the cab, the man begins ranting semi-coherently and coughing up blood, so the driver takes him to an emergency room. Soon after checking in, two men claiming to be from the CDC say that the patient is infected with a highly dangerous pathogen and must be taken away, but the man has already escaped with the taxi driver. Frank and Lt. Gebelhouse are asked by the CDC to assist in tracking then pair down before they can infect anyone else. In the course of catching the two, Frank comes in contact with Patient Zero's blood, exposing him to whatever disease the man is infected with. What Frank learns when the bloodstain is examined will reveal that a much larger conspiracy is taking place.

An retired couple traveling in a Winnebago stop for the night in the village of Bucksnort, Montana. Soon after pulling over, however, they are attacked and killed by a pack of wild dogs. The Millenium Group sends Frank to investigate, despite his protests that it's just an animal attack. When he arrives, however, the townsfolk behave strangely, particularly their insistence that Frank needs to be in before sundown. At the other extreme is a hyperactive California transplant who complains about a crazy old man living on the outskirts of his property. When Frank gets caught outside after sunset, the dogs make their return, attacking anyone they see except for an old man who appears to control them in some way.

As the Blacks are returning from their vacation in Virginia, Frank and Jordan go out to the parking lot while Catherine waits for their bags to arrive. As she waits, a man takes her photo and then abducts her, drugging her to keep her from fighting back. When he gets away, Frank learns that not only is this the man who has been harassing him with pictures of his family, but the Millennium group has known his identity long before this. As the kidnapper taunts Catherine with his rantings about how well he knows what's in Frank's mind, Frank must try to prevent his fear and range from clouding his abilities if he ever hopes to see his wife alive again.

The Blacks travel to Virginia to visit Catherine's family. After dinner, Catherine's father asks Frank to talk with a friend of his: the friend has been diagnosed with terminal cancer and his son is behind bars for murdering his wife. The prison will allow the son to visit his father, but the father has disowned him. At the urging of the mother Frank takes a look at the details of the case, and sees the young man does not fit the killing's profile, leading him to believe that the murder was committed by someone else. At the same time, another man has just murdered a suburban housewife and is taking her body up to an Appalachian campground, talking to it all the while. Could he have been involved in the earlier murder? Also, a shadowy figure continually appears, attempting to steer the murderer toward Catherine Black. What are his plans?

The episode opens at Chernobyl nuclear power plant just before the disastrous meltdown. As the plant crew are evacuating, one of the guards claims the meltdown was caused by someone named Yaponchik, who is seen climbing over a fence and escaping just before the explosion. Several years later, the same man approaches a stranger outside a Russian Orthodox church in New York and shoots him in the face. Frank and Peter are then called in to help the NYPD, who are also receiving help from an officer of the Moscow police. The Russian officer dismisses the claims about Yaponchik, saying he's just a fairytale, but Frank notices things that tell him otherwise. As he digs deeper, he finds signs linking Yaponchik to the Anti-Christ in the Book of Revelations.

Frank arrives in North Dakota to investigate a series of horse murders. Normally the Millennium Group would not get involved in this, but the killings have a ritualized element to them and Frank fears that the killer may soon escalate his activities. He encounters resistance from farmers who react with disgust to his analysis of the killer's sexual motivations, and from local veterinarian Dr. Vaughn who is disgusted with the hypocrisy of farmers who mistreat and slaughter their horses asking for help when someone else kills them. Although Frank hopes to head off the killer before any human lives are lost, the man is already starting to shift his fantasies from horses to women.

In the parking lot of a supermarket, a young man shoots a well-dressed businessman after making a strange speech. As Frank watches the event, however, it appears that the young man killed his target with a bolt of lightning rather than a bullet. The scene then flashes back several days earlier, with Frank being called in to help with a series of very elaborately staged satanic murder rituals. A rather dull-witted man with no history of violent crime is arrested for the crimes and then confesses at his bail hearing to killing Bob Bletcher as well. As this is going on, the defendant's lawyer, Al Pepper, keeps hounding Frank to leave the Millennium group and work for him. Frank, however, senses something very wrong with the slick lawyer.

Ephraim Fabricant, a doctor jailed for a string of murders that Frank Black helped solve, is wheeled into an operating room to donate a kidney to his ailing sister. As he recovers in post-op, someone posing as a nurse savagely beats the officer on guard and takes Fabricant away. As Frank and Peter try to locate him, their investigations lead them to Lucy Butler, a woman who had kept up a long correspondence with Fabricant and who now claims to be his wife. Several days later, a much weaker Fabricant is dumped at a nearby hospital, missing his second kidney. He warns Frank that there is another force at large, one which has terrified him and is now turning its attention toward Frank and his family.

Frank is found in an alley in Yakima covered in cuts and bruises and unable to remember where he is or how he got there. All he has are vague memories of a medical study, an email exchange with a doctor that he conducted under a false name, and the belief that someone has been killed. With Peter's help, Frank retraces his steps, first contacting the shut-in former doctor who introduced him to the man conducting the trials. From there, however, the trail seems to run cold: nobody at the clinic knows anything about a medical trial, and none of fellow guinea pigs that Frank contacts can come up with a consistent story about anyone being killed.

Frank leaves on a business trip to Utah, where he has been asked to create a profile of William Garry, a man who has murdered his wife and three children. Garry has already been convicted, and now the prosecutor needs Frank's profile to show he deserves the death penalty. Garry, who has put up no defense and says himself that he deserves death, describes the killings in perfect detail, claiming he acted out on a rage that had been building up inside him for years. As Frank investigates the crime scene, however, an image appears of a killing done with absolute calm. Some piece of the puzzle is missing, but few involved in the investigation want to look for it.

Frank's brother Tom comes to Seattle for his baby's baptism, for which Frank is the godfather. Immediately after the ceremony, however, Tom's wife is kidnapped right outside the church. With the case coming so close to home, Lt. Bletcher warns Frank to stay away, as any evidence he uncovers will be considered biased and tainted. Frank, however, is unable to just sit by and do nothing when the victim is a member of his own family. Perhaps more disturbing to Frank and Catherine is the fact that Jordan knew her aunt was in trouble when nobody else could see her, indicating that she may have inherited Frank's terrible talent.

In the end, however, chemical analysis of the drugs found in the victims' bodies produces the concrete results that point to the killer.

Frank helps out Bob on a strange case: a woman receives a package containing a human tongue, with no possible explanation of who sent it or why. Bob reveals that this is just one of a series of similar incidents, usually involving hands or fingers. A little later a body is found missing its tongue, and Frank soon matches it with another corpse belonging to the murderer. While Bob is willing to call this a closed case, Frank believes that the truth goes deeper and there are more people involved. Bob's skepticism evaporates when the gruesome packages start arriving once again.
